چکیده

PURPOSE Phosphorus magnetic resonance spectroscopy ((31)P-MRS) affords unique insight into cardiac energetics but has a low intrinsic signal-to-noise ratio (SNR) in humans. Theory predicts an increased (31)P-MRS SNR at 7T, offering exciting possibilities to better investigate cardiac metabolism. We therefore compare the performance of human cardiac (31)P-MRS at 7T to 3T, and measure T1s for (31)P metabolites at 7T. METHODS Matched (31)P-MRS data were acquired at 3T and 7T, on nine normal volunteers. A novel Look-Locker CSI acquisition and fitting approach was used to measure T1s on six normal volunteers. RESULTS T1s in the heart at 7T were: phosphocreatine (PCr) 3.05 ± 0.41s, γ-ATP 1.82 ± 0.09s, α-ATP 1.39 ± 0.09s, β-ATP 1.02 ± 0.17s and 2,3-DPG (2,3-diphosphoglycerate) 3.05 ± 0.41s (N = 6). In the field comparison (N = 9), PCr SNR increased 2.8× at 7T relative to 3T, the Cramer-Ráo uncertainty (CRLB) in PCr concentration decreased 2.4×, the mean CRLB in PCr/ATP decreased 2.7× and the PCr/ATP SD decreased 2×. CONCLUSION Cardiac (31)P-MRS at 7T has higher SNR and the spectra can be quantified more precisely than at 3T. Cardiac (31)P T1s are shorter at 7T than at 3T. We predict that 7T will become the field strength of choice for cardiac (31)P-MRS.

Adiabatic excitation for 31P MR spectroscopy in the human heart at 7 T: A feasibility study

PURPOSE Phosphorus magnetic resonance spectroscopy (31 P-MRS) provides a unique tool for assessing cardiac energy metabolism, often quantified using the phosphocreatine (PCr)/adenosine triphosphate (ATP) ratio. Surface coils are typically used for excitation for 31 P-MRS, but they create an inhomogeneous excitation field across the myocardium, producing undesirable, spatially varying partial sa...

Bloch‐Siegert B1+‐mapping for human cardiac 31P‐MRS at 7 Tesla

PURPOSE Phosphorus MR spectroscopy ((31) P-MRS) is a powerful tool for investigating tissue energetics in vivo. Cardiac (31) P-MRS is typically performed using surface coils that create an inhomogeneous excitation field across the myocardium. Accurate measurements of B1+ (and hence flip angle) are necessary for quantitative analysis of (31) P-MR spectra. We demonstrate a Bloch-Siegert B1+-mappi...

Human Cardiac 31P-MR Spectroscopy at 3 Tesla Cannot Detect Failing Myocardial Energy Homeostasis during Exercise

Phosphorus-31 magnetic resonance spectroscopy (31P-MRS) is a unique non-invasive imaging modality for probing in vivo high-energy phosphate metabolism in the human heart. We investigated whether current 31P-MRS methodology would allow for clinical applications to detect exercise-induced changes in (patho-)physiological myocardial energy metabolism. Hereto, measurement variability and repeatabil...

Myocardial skeletal muscle signal spoiling using a crusher coil: a human cardiac phosphorus (31P) MR spectroscopic imaging study at 7 Tesla

Background P-MRS provides direct insights into myocardial energy supply (ATP, ADP, phosphocreatine (PCr) and inorganic phosphate). An initial study demonstrated that 7T cardiac P-MRS has 2.8x greater SNR than at 3T. However, the translation of more sophisticated P-MRS protocols to 7T is particularly challenged by increased RF heating of tissue at 7T. Chen and Ackerman introduced the surface spo...

7T versus 3T phosphorous magnetic resonance spectroscopy in patients with dilated cardiomyopathy

Background Phosphorous magnetic resonance spectroscopy (PMRS) allows insight into cardiac energetics in vivo, but it is a technique with an intrinsically low signal to noise ratio (SNR). The 2.4x increased SNR, that is predicted by theory at 7T compared to 3T, should allow detection of smaller changes in metabolite concentrations or measurement of changes in smaller patient groups, which will f...
